Te Tumu Waiora - te reo for 'to head towards wellness' - is a new model of healthcare, which aims to provide all New Zealanders experiencing challenges with access to convenient, high quality, integrated and person-centred care and support. The model has been rolled out nationally.
Odyssey House is an established NGO working with people around mental health, addictions and wellbeing. We work within the Te Tumu Waiora collaboration, providing resources on mental health and wellbeing as part of a general practice team.
The health coach role aims to help patients and their whanau gain knowledge, skills and confidence to better manage their health and wellbeing. This includes supporting patients and whanau to set goals and work towards them, manage long term conditions and linking them with appropriate community supports and health providers. The role involves being an integral part of the general practice team, working directly with whāiora (in search of wellbeing) and their whānau to help improve their mental health and wellbeing.
This role is based part-time in Christchurch across two practices.
Successful applicants will receive comprehensive training and ongoing mentoring.
We are looking for someone who has:- Experience in health and preferably mental health, particularly supporting and motivating people to improve their own physical health and wellbeing
- A minimum of a relevant level 4 qualification in health and wellbeing
- The ability to connect with a wide range of individuals and whānau from various backgrounds and cultures
- Excellent communication skills and a flexible, positive, and adaptable attitude
- The ability to adhere to evidence-based models and practices
- Capable of managing their own workload and working both autonomously and collaboratively within a busy general practice environment
- Willingness to work with groups and one on one
